I agree -- this is the quickest way to stuff up a character.  You need
to know a lot about a world before committing to character details.
Otherwise, the profile 'examiners' have to either accept contradiction
with existing world details, a multi-staged feedback process before
allowing the new character into the world, or vague and world-generic
profiles omitting anything that might conflict with in-game culture.

I fully agree with this point.  Children are expected to get things
wrong and have to find out from older people.  Having native adults
get basic cultural things wrong can be really annoying, both to the
new player and to more experienced ones.

The other side of the coin is the prevalence in existing MU*s for the
in-game culture to be completely different from the documented
culture.
